Why does the icon for the fallback application menu not appear for some applications? [on hold]

I was wondering what the reason some apps do not display their icon for the fallback app menu was. What I refer to is the leftmost icon in each headerbar. In the screenshot, Web has its icon, but Maps displays a diamond with gears in it. I assume that this icon is a placeholder for any executable, since other apps such as Photos and Contacts show the same icon.
